The form below is the 2011-2012 Medical Release Form for West Jackson Student Ministry Events including the following:

• 48 Hours - November 4-6, 2011
• Hub City Weekend - March 2-4, 2012
• JH Splash - June 3-7, 2012
• SH Camp - June 24-29, 2012

One form per student will cover all of these events. However, if there are any changes in your medical insurance or your child's medical history/needs, you will need to complete a new form so we can make sure we have your most updated information.
