To access a Passport application, go to www.travel.state.gov and follow the following instructions:

• Click on "Passports"
• Click on "Fill Out Form DS-11: Application for a U.S. Passport"
• Click on "Fill Out Form DS-11: Application for a U.S. Passport" again
• Scroll down and click on "Print a Blank Form DS-11"
• You can fill out the application but MAKE SURE YOU DO NOT SIGN YOUR APPLICATION UNTIL YOU GET TO THE POST OFFICE DOWNTOWN.

You will need the following as you turn in your application at the Post Office downtown which is open from 10 am to 3pm Monday-Friday:

• Proof of U.S. citizenship/Certified copy of Birth Certificate
• Proof of Identity/Drivers License (if you are a minor, your parents will need to show proof of their identity)
• Two photographs (these can be taken at the Post Office or at Walgreens. Just tell them you need passport photos)
• Money for fees.

PASSPORTS ARE DUE in our office by Thursday, April 7th. At the meeting on that night, we will collect everyone's passports and make copies to keep on file.
